Here you should find some of the necessary paper work for students and parents at St. Louis School.
New Student Pre-Registration Form 
This form is for any student entering or returning to St. Louis School. This form must be completed and sent to the school before your student is registered. You should send a copy of your child's baptismal certificate which will be returned to you, which will be sent back to you. A copy of immuniziations and the child's medical form must be on file before the first day of school.
